For Trust Beneficiaries
If you are a current Future Care Trust beneficiary and have a disbursement request please submit via email at trustservices@futurecareplanning.org or by postal mail: to Future Care Planning Services Trust Department 1000 Elmwood Avenue Suite 500 Rochester, NY 14620. A trust representative will review and reach out to you via phone or email with any questions regarding your request.Trust deposits may still be mailed directly into our office or be set up as an automatic bill payment through your banking institution.

Tax Information
If you have a First Party Lump Sum or Third Party Lump Sum Trust account or if you were a recipient of gift funded by the Golisano Life Enhancement Fund, The annual K1 statements were mailed to each person who had received a gift funded through the Golisano Life Enhancement fund. If you file taxes you may include this statement with your tax-filing documents. If you have questions regarding the K1 statement we encourage you to share the statement with your tax preparer.  

The Golisano Life Enhancement Fund (GLEF) is open for 2024. Please be sure to use application with revision date of January 2024.  Completed applications and supporting documentation can be emailed to: golisanofund@futurecareplanning.org 

or faxed to 585- 210-4048 or mailed to: Future Care Planning Services Attn. GLEF 1000 Elmwood Avenue Rochester, NY 14620.
